At Meriden and Wallingford Veterinary Associates, we are always accepting new clients. In an attempt to serve you better, and to alleviate the paperwork typically accompanying your first visit, we have provided a form that can be filled out and submitted online. If you wish, you may also print out the form and bring it along with you to your pet’s visit.

If so, we’d love to have your new address. Not only does this cut down on paperwork during office visits, but it also allows us to send you timely updates and reminders for vaccinations and checkups.

RX Refills

Looking to refill your pet’s prescription? This form allows you to start the process before your visit. Our in-house pharmacy is convenient, well stocked, and ready to serve your needs.
